“Bumps Around the Eyes: What Are They? ③Could it be Eccrine Hidrocystoma?”

There are various conditions that can cause bumps around the eyes. In previous blogs, we discussed syringoma and milia. This time, our focus is on eccrine hidrocystoma.

Eccrine hidrocystoma is a benign skin tumor derived from sweat glands, particularly appearing in the facial region. Here, we delve deeper into the specifics of eccrine hidrocystoma.

Characteristics of Eccrine Hidrocystoma

Eccrine hidrocystoma manifests as cystic structures in the sweat glands of the skin. These lesions are typically benign and are most commonly seen around the eyes. They usually appear as small, dome-shaped, clear or bluish cysts on the skin, ranging in size from a few millimeters to about a centimeter. While these lesions are often solitary, there can be instances of multiple cysts. One distinguishing feature of eccrine hidrocystoma is its transillumination, where the cysts allow light to pass through, rendering them a bluish appearance when viewed against light.

 

 

Causes of Eccrine Hidrocystoma

While the exact cause and mechanism of eccrine hidrocystoma’s development remain unclear, it’s believed that these cysts form due to blockage or dilation of the eccrine sweat gland ducts. Everyday skin injuries or sun exposure may play a role in its occurrence. Additionally, excessive heat or humidity could also heighten the risk.

Histologically, these tumors are characterized by cystic structures lined with a double layer of cuboidal epithelium, filled with a clear liquid resembling the sweat produced by eccrine glands. Surrounding tissues might show signs of inflammation or fibrosis, based on the lesion’s duration and severity.

 

Frequency of Occurrence

Though relatively rare, eccrine hidrocystomas are generally seen in adults and the elderly. While women tend to be more affected, men can also develop the condition. However, gender prevalence might vary depending on studies.

 

 

Symptoms

These cystic tumors typically appear as small, clear or pale-blue bumps (from a few millimeters to a centimeter) particularly around the eyes. They are usually painless but can cause visual disturbances or discomfort.

 

 

Differentiating Between Eccrine Hidrocystoma and Syringoma

 A similar bump that might appear around the eyes is the syringoma. However, they can be distinguished: syringoma are semi-transparent and flesh-colored, while eccrine hidrocystomas are clear or bluish. By touch, syringoma are firm, while eccrine hidrocystomas are soft and mobile.

Treatment Options for Eccrine Hidrocystoma

■Observation: Since they’re usually benign and symptomless, treatment might not be necessary.

■Surgical Excision: These cysts can be surgically removed.

■Laser Treatment: Lasers can treat eccrine hidrocystoma similarly to surgical removal.

■Aspiration: Draining the cyst can reduce its size, though recurrence is possible.

■Botox: Botox (botulinum toxin) can potentially prevent or treat the cyst formation and symptoms by reducing sweat production.

 

 

Conclusion

Like many other benign facial bumps, eccrine hidrocystomas might not require removal. However, if you’re concerned about them from a cosmetic standpoint, especially if multiple lesions arise, it’s recommended to consult with an experienced cosmetic surgeon.
